To Join, print and fill out this form,
add your dues check for
$25 made out to IJC,
and mail to IJC, c/o Bedell, 6712 S. 425 E., Lebanon, IN 46052
The dues of $25 per household covers all persons
living in the same house.
Dues are due on September 1 of each year.
Failure to pay dues in September may cause membership
cancellation
and having name removed from the mailing list for Jazz Notes.
